Get a Lifestyle Score based on the healthy choices you make each day plus a scientific picture of your heart health! Answer questions about your nutrition, activity, sleep and health history to see where you stand and how you can improve to be the best you. The more info you give, the more accurate your scores and recommendations will be. It takes less than 10 minutes to complete.

How do I access it?

Website: The HA can be accessed directly in your Choose Well account powered by Virgin Pulse from the Programs Page under the Programs tab.

App: Click on Health Assessment under the main menu on the app

Strike Up a Conversation with your Provider

Strike Up a Conversation with your Provider

One of the most important things that you can do for your well-being is to visit your health care provider annually to set and discuss your health and wellness goals. By taking time to meet with your provider, you will be rewarded with extra points in your Choose Well account. Provider Care visits between November 1, 2017 thru November 6, 2018 will be accepted.

How do I sumbit proof of your provider visit?

  1. Login into your Choose Well account
  2. Click on Programs
  3. Go to Strike Up a Conversation with Your Provider
  4. Click on Start Now button
  5. Complete the online attestation form by filling out your Provider namd and date completed
  6. Click the consent
  7. Click on Submit button
  8. Recieve 15,000 points

Follow Your Dreams Challenge: March 12th through March 25th

Everyone deserves a dream vacation! In this challenge you will daydream from one amazing destination to another, all while learning about the importance of sleep: why you need it, how to get more of it and how improve it. The Follow Your Dreams Challenge will allow you to relax and enjoy unique and fantastical destinations around the world from the comfort of your own home.

Daily Cards

Daily Cards

Your Daily Cards will show you personalized content to learn and explore that is driven by what you've selected as your goals and interests in "My Interests". You'll get two new cards each day with new content. Completion of each card is worth 20 Points each (up to 40 Points per day).

To complete your Daily Cards:

1) Log into your Choose Well account powered by Virgin Pulse.

2) Your two Daily Cards will be displayed on your home page over your cover photo. Click on the "Daily Cards" icon in the top left corner below the logo.

3) Click on the arrow icons to the left or right of the cards to scroll between them.

4) Once you've read the content on your card, click on the "Got It!" button to complete.

Note*: If you enjoy the content on this card, you also can click the thumbs up icon to let us know!

5) Click on the right arrow icon to move to the next card.

6) Once you've completed both of your Daily Cards, the home page will display with both cards as completed with green checkmarks. You can revisit the content of these cards at any time during the current day by clicking on the card!

Note*: Your daily cards are only available for 24 hours, and will switch at the end of every day. Unfortunately, we are unable to retrieve the content of your past cards, as it is unique to each member, and randomized based on your selected "Interests".

7) Make sure to check back each day to complete your cards and receive up to 40 Points!

It's important to complete "My Interests" to identify the important areas of well-being that you'd like to focus on during your journey to a better self with the Virgin Pulse program. These interests drive the content provided on the Daily Cards that you complete each day to help form healthy habits.

To complete "My Interests", follow these steps:

1) Log into your Choose Well account on the Virgin Pulse member website.

2) Hover over your profile picture in the right corner of the top-navigation menu. In the drop-down menu that appears, select “My Interests”.

3) A page overlay will appear that displays "My Interests".

4) To select an interest, click the circle icon above the interest title. You can select from the nine following options:

  • Getting Active
  • Eating Healthy
  • Sleeping Well
  • Reducing Stress
  • Building Relationships
  • Contributing to My Community
  • Learning New Things
  • Managing My Finances
  • Being Productive

Note*: You can select as few or as many interests as you'd like. At any time after re-evaluating your objectives, you can change your interests to ensure that you're focusing on the behaviors that make sense for you.

5) Click on the "Save" button.

6) The page overlay will be removed, and your changes will be saved.

7) Starting tomorrow, your Daily Cards will display content that is associated to your interests!

It's Better Together

It's Better Together

Invite your family, friends and collegues to keep you motivated and supported. You can add as many caregivers within our organization as you want and up to 10 friends or family members to be your friend. When you add a friend, they will receive a notification on their Virgin Pulse website, and can accept your request.

To add Friends, follow these steps:

1) Log into your Choose Well account.

2) Hover over “Friends” in the top-navigation menu. In the drop-down menu that appears, select “My Friends”.

3) On the "My Friends" page, click on the "Add Friends" button in the top right corner.

4) A pop-up will appear where you can view Suggested Friends or search for Friends by name.

5) Your Suggested Friends will display other members within your sponsor that you are not friends with yet, that have the highest amount of mutual friends with you. To add a suggested friend, click the "Add Friend" button below them.

6) To search for friends by name, click on the "Find by Name" tab in the pop-up. Start typing in a person's first name or last name (at least three characters in order for search to begin working). Your search results will begin to populate below as you type.

7) To add a friend from your search, click on the "Add Friend" button to the right of their name.

  • If you're already friends with a person in your search results, it will display "Friends" next to their name.
  • If you have already sent them a friend request, it will display "Invite Sent".
  • If the person has sent you a friend request and you have not yet responded, there will be a checkmark icon and an "X" icon next to their name, and you can accept or deny their friend request in this view.

8) Once a person has accepted your friend request, they will begin to display on your Friends Leaderboard and your "My Friends" page. You'll be able to view their profile and invite them to personal challenges!

There are plenty of ways to earn points for being active. Wear your activity tracking device or use your tracking mobile application that is compatible with the Choose Well platform to track your daily activity and earn rewards for Steps or Active Minutes. (Check out the list of compatible devices/apps here)

  • More than 1,000 steps in a day will earn 10 Points.
  • More than 2,000 steps in a day will earn 20 Points.
  • More than 3,000 steps in a day will earn 30 Points.
  • More than 4,000 steps in a day will earn 40 Points.
  • More than 5,000 steps in a day will earn 50 Points.
  • More than 6,000 steps in a day will earn 60 Points.
  • More than 7,000 steps in a day will earn 70 Points.
  • More than 8,000 steps in a day will earn 80 Points.
  • More than 9,000 steps in a day will earn 90 Points.
  • More than 10,000 steps in a day will earn 100 Points.
  • More than 11,000 steps in a day will earn 110 Points.
  • More than 12,000 steps in a day will earn 120 Points.
  • More than 13,000 steps in a day will earn 130 Points.
  • More than 14,000 steps in a day will earn 140 Points.
  • More than 15 Active Minutes in a day will earn 70 Points.
  • More than 30 Active Minutes in a day will earn 100 Points.
  • More than 45 Active Minutes in a day will earn 140 Points

Or, you can earn rewards for workoutsessions using your FitBit, Polar device, certain Garmin devices, Apple Watch, or Strava mobile application.

  • 15 minute Workout will earn 70 Points.
  • 30 minute Workout will earn 100 Points.
  • 45 minute Workout will earn 140 Points.

Note*: 140 Points is the maximum daily reward for step/active minutes/workout session activity. You will not be rewarded for both steps and active minutes and workout sessions, only the one with the higher Points value of the three.

You also can earn Activity Trophies for your level of activity each month!

  • Track Steps 10 Days in a Month: Track your steps using a validated activity tracker for any 10 days within a calendar month for 100 Points per month.
  • Track Steps 20 Days in a Month: Track your steps using a validated activity tracker for any 20 days within a calendar month for 200 Points per month.
  • Take 7,000 Steps 20 days in a Month: Track your steps using a validated activity tracker for any 20 days with more than 7,000 steps within a calendar month for 400 Points per month.
  • Take 10,000 Steps 20 days in a Month: Track your steps using a validated activity tracker for any 20 days with more than 10,000 steps within a calendar month for 500 Points per month.
